Health Savings Account (HSA) Authorization

Purpose

If you are already enrolled in a HSA-qualified health plan, this form allows you to enroll, change or terminate your HSA deductions.

If the form is submitted by the 10th of the month, it will be effective for that month's pay cycle. Otherwise, it will take effect the following month.
