- The voluntary Anthem Blue View Vision Plan covers annual eye exams for a $20 copayCopayment (copay)A fixed-dollar amount that you must pay out of your pocket at the time of service to a provider or a facility for a specific health covered service. Copays do not apply to the deductible requirement. For example, an office visit may have a copay of $30 under the Exclusive Plan and $40 under the Extended. You must pay the amount at the time of service., as well as $225 toward the yearly purchase of frames or contact lenses, for each participant. CU Health Plan - Kaiser covers routine vision exams and routine eye exams (eye refractions) provided by Kaiser network optometrists ($30 copay) or ophthalmologists ($40 copay) to determine the need for vision correction and to provide a prescription for eyeglasses or contact lenses are covered.
